Make a meaningful impact in patient care by conducting home-based health risk assessments in Benton County. This per diem opportunity offers Nurse Practitioners the flexibility to design their own schedules while providing essential healthcare services to Medicare Advantage members in Kennewick, Richland, and West Richland.

Schedule:

• Flexible shifts available between Monday and Sunday
• Self-scheduling to accommodate your availability
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage plan participants in their homes
• Review and evaluate prior and current medical history, including all medical conditions and medication use
• Educate patients about their health conditions and prescribed medications
• Identify any gaps in care and recommend steps for improvement
• Visits may cover annual health assessments or follow up after a hospital discharge
• Note: This role does not include developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Washington State
• Current CPR certification (BLS)
• Board Certification through the AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• At least 6 months of recent Nurse Practitioner experience preferred
