Empower patients by delivering high-quality care through comprehensive in-home health risk assessments. As a Nurse Practitioner in a per diem role, youll enjoy the autonomy of creating your own schedule with flexible shifts available any day of the weekMonday through Sunday. Help support Medicare Advantage plan members throughout Bonneville County, ID, including Idaho Falls, Ammon, and Iona, with your clinical expertise and compassionate approach.
Schedule:
- Enjoy the flexibility of per diem shifts between Monday and Sunday

- Minimal weekly or bi-weekly hourly commitments required
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members in various home settings
- Assess patients prior and current medical histories, underlying conditions, and medication use
- Deliver clear patient education regarding diagnoses and medication management
- Identify care gaps and offer recommendations, promoting improved overall health
- Focus solely on assessmentsno need to create ongoing treatment plans or prescribe medication
Qualifications:
- Valid Nurse Practitioner license in Idaho
- Current BLS/CPR certification
- Board certified via AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
