Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members within Bend, Redmond, Deschutes River Woods, Three Rivers, La Pine, Sisters, and nearby areas
• Assess patients prior and current medical history, ongoing medical conditions, and medication use
• Deliver education to patients regarding their health conditions and related medications
• Identify gaps in care and offer thoughtful recommendations to improve patient outcomes
• Focus solely on evaluation; no prescribing or management of treatment plans required

Qualifications:

• Active and unrestricted Nurse Practitioner license in Oregon
• Current CPR certification (BLS)
• Board certified through AANP or ANCC
• At least six months of recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
