Job Description
Deliver essential healthcare in the community by performing home health risk assessments for Medicare Advantage members residing throughout Robeson County. This per diem opportunity is ideal for an experienced Nurse Practitioner seeking flexibility and meaningful patient interaction through one-on-one care in the home setting.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities include:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review medical and medication histories, as well as prior and current medical conditions
- Educate patients on conditions, medications, and general health management
- Identify gaps in care and provide actionable recommendations
- Administrative responsibilities related to documentation and mileage tracking
- No prescribing or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC
- Current BLS certification
- At least 6 months of Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
You can expect competitive compensation of $80-$90 per evaluation, in addition to a mileage stipend and compensation for administrative tasks associated with the assessments.
