Advance your clinical expertise as a Nurse Practitioner performing home health risk assessments for Medicare Advantage plan members within Robeson County, NC. In this per diem opportunity, youll play a vital role in promoting patient well-being by evaluating health status and providing essential health education during home visits. Assessments average 45 minutes and may focus on annual health checkups or occur after recent patient discharge. Scheduling is highly flexible, with the option to create your own hours and select shifts between Monday and Sunday. You may be assigned visits within various towns across Robeson County, including Lumberton, Red Springs, and Pembroke.
Schedule:
- Flexible shifts available Monday through Sunday
- Control your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Primary Responsibilities:
- Complete in-home annual and post-discharge health risk assessments for Medicare Advantage members
- Review and document prior/current medical histories, chronic conditions, and medications
- Deliver patient education regarding diagnoses, preventive measures, and medications
- Identify and report any gaps in care, offering actionable recommendations
- No prescribing of medications or development of treatment plans required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of current nurse practitioner experience preferred
Youll be compensated at $80-$90 per completed evaluation, with an additional mileage stipend and pay for administrative time.
