Job Description

Make a meaningful impact by delivering in-home health assessments as an experienced Nurse Practitioner. This per diem opportunity allows you to utilize your clinical expertise with the convenience of flexible schedulingperfect for professionals seeking autonomy and variety in their workday.

Schedule:

  • Flexible shifts available any day of the week, Monday through Sunday
  • Design your own schedule to fit your personal and professional priorities
  • Minimum weekly or bi-weekly hourly commitmentschoose what works for you

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Assess and document prior and current medical history, ongoing medical conditions, and medication use
  • Provide patient-centered education on health conditions and prescribed therapies
  • Identify care gaps and recommend actions to further patient well-being and care continuity
  • Perform annual assessments and post-discharge visits; no treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • Current CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of NP experience strongly preferred

This role is ideal for Nurse Practitioners who value flexibility in their professional life, want competitive pay, and prefer engaging directly with patients in their homes. Compensation includes a rate of $80-$90 per evaluation, with additional reimbursement for mileage and administrative time.

The work environment will take you throughout Scotland County, including Laurinburg, Wagram, and Gibson. Each 45-minute visit is designed to support the health and wellness of Medicare Advantage plan members.
