Job Description

Deliver vital healthcare directly to patients in their own homes as a Nurse Practitioner providing home health risk assessments. This per diem opportunity is perfect for professionals who value flexibility and autonomy, with the ability to create your own schedule and choose your shifts between Monday and Sunday. Shifts are available across Kay County, including Ponca City, Blackwell, Newkirk, and Tonkawa, allowing you to make a meaningful difference across a broad community.

Schedule:

  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Assess patients prior and current medical history, conditions, and medication regimens during in-home visits (approximately 45 minutes per evaluation)
  • Educate patients on their medical conditions and prescribed treatments
  • Identify gaps in care and recommend resolutions to support optimal health outcomes
  • No diagnosis, formulation of treatment plans, or prescribing necessary

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Current BLS (CPR) certification
  • Board certification through AANP or ANCC required
  • Preferred: At least 6 months of recent Nurse Practitioner experience
  • MSN degree from an accredited nursing program
