Step into a rewarding advanced practice provider opportunity delivering compassionate care to Medicare Advantage plan members through in-home health risk assessments. This per diem position offers exceptional flexibility for Nurse Practitioners who enjoy autonomy and meaningful patient interactions within Greene County, NYincluding the Catskill, Cozsackie, Ashland, and Ravena areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingset your own hours
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct in-home health risk assessments (approx. 45 minutes each) for Medicare Advantage members
- Review and evaluate patients previous and current medical histories, conditions, and medication use
- Provide patient education on diagnosed medical conditions and prescribed medications
- Identify potential gaps in care and make clinical recommendations to ensure continuity and safety
- No treatment plans or medication prescriptions requiredcomprehensive assessments only
Qualifications:
- Active Nurse Practitioner license in New York
- Graduate of an accredited MSN program
- Board certification as an NP (AANP or ANCC)
- Current CPR certification (BLS)
- At least 6 months of NP experience preferred, but not mandatory
- Home health risk assessment or similar in-field clinical experience valued
