Deliver personalized healthcare in a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This role serves Medicare Advantage plan members throughout Greene County, NYincluding Catskill, Cozsackie, Ashland, Ravena, and surrounding areas. The position offers the autonomy to create your own schedule, allowing for outstanding flexibility.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own weekly work schedule
- Meet a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage health plan members
- Review and evaluate medical history, current health conditions, and medication regimens
- Offer education to patients regarding their conditions and prescribed medications
- Identify gaps in care and develop recommendations to address them
- No ongoing patient management, treatment planning, or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in New York
- Current CPR (BLS) certification
- Board Certification via AANP or ANCC
- Minimum of 6 months recent NP experience (preferred)
- Master's degree (MSN) from an accredited nursing program
For each 45-minute evaluation completed, compensation ranges from $80-$90, along with a mileage stipend and additional pay for administrative time.
