Deliver high-quality patient care and support community wellness as a Nurse Practitioner providing home health risk assessments. This per diem opportunity allows you to make a difference for Medicare Advantage members throughout Pacific County, focusing on holistic patient evaluations in residential settings. Enjoy the flexibility to design your own work schedule, with the option to pick up visits in areas such as Raymond, South Bend, and Long Beach.

Schedule:

• Flexible shifts available between Monday and Sunday
• Create your own schedule with no fixed hours
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
• Assess patients prior and current medical history, medical conditions, and medication use
• Educate patients on managing medical conditions and medications appropriately
• Identify and address gaps in care, providing actionable recommendations
• Visits typically last approximately 45 minutes, varying between annual evaluations and post-discharge assessments
• No treatment planning or prescribing required; this is a purely assessment-based role

Qualifications:

• Active Nurse Practitioner license in Washington State
• Board Certification through AANP or ANCC
• Valid and current CPR (BLS) certification
• 6 months or more of recent nurse practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
