Deliver critical healthcare insights as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members across Nodaway County, Missouri. This per diem position supports patients living in Maryville, Burlington Junction, Hopkins, and surrounding areas, ensuring comprehensive evaluations and continuity of care from the comfort of their own homes.
Schedule:
- Flexible shifts available throughout the week (MondaySunday)

- Commitment to a minimum weekly or bi-weekly number of hours
Key Responsibilities:
- Conduct 45-minute home health risk assessments for Medicare Advantage enrollees
- Review prior and current medical history, ongoing health conditions, and medication use
- Educate patients about their diagnoses and medication regimens
- Identify gaps in care; provide actionable recommendations to help resolve them
- Focus solely on assessmentsno treatment plan creation or medication prescribing required
Qualifications:
- Licensed as a Nurse Practitioner in Missouri
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- At least 6 months of recent nurse practitioner experience (preferred)
- Master of Science in Nursing (MSN) from an accredited program
