Job Description

Nurse Practitioners with a passion for home health have a unique opportunity to make a difference by performing risk assessments for Medicare Advantage members throughout Kootenai County, ID. This per diem role offers a highly flexible schedule and the chance to create lasting impact through comprehensive patient evaluations in a home setting.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual and post-discharge home health risk assessments for Medicare Advantage members living in communities such as Coeur dAlene, Post Falls, and Hayden
  • Review and assess patients prior and current medical history, ongoing conditions, and medication usage
  • Provide thorough patient education regarding medical conditions and medications, ensuring clear understanding
  • Identify care gaps and recommend solutions for improved wellness
  • Focus solely on assessmentsno development of treatment plans or prescribing of medications required

Qualifications:

  • Active Nurse Practitioner license in the state of Idaho
  • Board Certified through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN from an accredited nursing program
  • At least 6 months of NP experience preferred (newer NPs are welcome to apply)

This position offers a competitive structure, with $80$90 compensation for each evaluation completed, plus stipend for mileage and administrative time.
