Job Description

This per diem position involves performing home health risk assessments with Medicare Advantage plan members throughout Chautauqua County, including Jamestown, Dunkirk, Chautauqua, and Mayville. Assessments are scheduled per your availability, offering you control over work-life balance and flexibility around your other professional or personal commitments.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete annual health risk evaluations for Medicare Advantage members in a variety of home settings
  • Assess patients medical histories, current medical conditions, and medication profiles
  • Deliver thorough education about medical conditions and medications, empowering patients in their health
  • Identify gaps in care and recommend follow-up, with no requirement for treatment planning or medication prescribing

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited university
  • Current CPR certification (BLS)
  • At least 6 months of Nurse Practitioner experience preferred
