Deliver impactful care directly to patients in their homes by conducting comprehensive risk assessments for Medicare Advantage plan members throughout Guilford County. This per diem opportunity gives Nurse Practitioners the ability to design flexible schedules, with shifts available any day of the week and no required minimumsperfect for those seeking autonomy and meaningful patient connections.
Schedule:
- Flexible shifts, Monday through Sunday
- Self-selected scheduling to meet your availability
- Minimum commitment options available weekly or bi-weekly
Key Responsibilities:
- Perform thorough home health risk assessments with each visit averaging 45 minutes
- Review patient medical history, current conditions, and medications
- Educate members about their health conditions and prescribed medications
- Detect gaps in care, providing actionable recommendations where needed
- Focus assessments mainly on annual check-ups or post-discharge evaluations
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Master of Science in Nursing (MSN) from an accredited program
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- Six months of recent nurse practitioner experience preferred
