Take the next step in your advanced practice nursing career by supporting at-risk populations through personalized home health risk assessments on a per diem basis. As a Nurse Practitioner specializing in home health, youll have the opportunity to provide essential care and health education to Medicare Advantage plan members throughout Randolph County, including Asheboro, Archdale, and Trinity. Youll play a pivotal role in the continuum of care, ensuring that patients needs are assessed and met during annual and post-discharge visits in the comfort and privacy of their homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule for maximum work-life balance
• Minimum hourly commitment required on a weekly or bi-weekly basis

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments and post-discharge evaluations for Medicare Advantage members
• Review and document prior and current medical history, health conditions, and medication use
• Educate members on their medical conditions and prescribed medications, promoting better health outcomes
• Recognize and document any gaps in care, offering recommendations for follow-up or further evaluation as needed
• No requirement for development of treatment plans or prescribing of medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification via AANP or ANCC
• MSN degree from an accredited school of nursing
• Current CPR (BLS) certification
• At least 6 months of current NP experience preferred

Compensation:

• Earn $80$90 per evaluation, plus a mileage stipend and payment for administrative time
