Take on an impactful per diem opportunity as a Nurse Practitioner specializing in home health within Taos County. In this role, you will have the flexibility to create your own schedule, conducting home health risk assessments to support Medicare Advantage plan members in locations such as Taos, Questa, and Red River. The work focuses on thorough assessments and patient education, with each evaluation lasting about 45 minutes and involving annual health assessments or post-discharge visits.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform home health risk assessments for Medicare Advantage members within Taos County
• Review and evaluate prior and current medical history, conditions, and medication usage
• Educate patients on their medical conditions and prescribed medications
• Identify gaps in care and recommend solutions
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active and unrestricted Nurse Practitioner license in New Mexico
• Current BLS (CPR) certification
• Board Certification through AANP or ANCC
• A minimum of six months of NP experience preferred
• Master of Science in Nursing (MSN) from an accredited nursing program

As part of this per diem position, you will receive a competitive rate per evaluation, along with a mileage stipend and reimbursement for administrative time.
