Make a meaningful impact by supporting patients in the community through comprehensive home health risk assessments. This per diem opportunity is designed for a Nurse Practitioner who values flexibility, autonomy, and patient-centered care. As part of this role, you will visit Medicare Advantage plan members in Washington County, which includes Bartlesville, Dewey, Copan, and nearby areas. Each in-home evaluation averages about 45 minutes, with visits tailored to patient needs, including annual health assessments and post-discharge check-ins.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Create your own work schedule around other commitments
• Minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
• Assess prior and current medical conditions, review medical and medication history
• Educate patients about their medical conditions, medications, and available care strategies
• Identify any gaps in care and make recommendations to help close those gaps
• No responsibility for treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Oklahoma
• Board Certification through AANP or ANCC
• Current CPR certification (BLS required)
• MSN from an accredited nursing school
• Minimum of 6 months' recent experience as a Nurse Practitioner preferred
