Primary Responsibilities:

• Perform thorough annual health risk assessments in members homes, focusing on medical history, ongoing conditions, and medication use
• Deliver clear education on health conditions, medications, and preventive care measures
• Identify care gaps and share actionable recommendations to improve patient wellness
• Complete standardized reports for each visit; no creation of treatment plans or medication prescribing involved

Education & Experience Requirements:

• Active Nurse Practitioner license in NC
• Board certified (AANP or ANCC)
• MSN degree from an accredited nursing program
• Current CPR/BLS certification required
• Minimum of 6 months NP experience preferred, with an interest in independent home-based work
