Job Description

Take on a dynamic, patient-focused role conducting home health risk assessments for Medicare Advantage members across Wake County, including Raleigh, Cary, and Apex. As a Nurse Practitioner in this per diem position, youll use your expertise to deliver comprehensive evaluations in patients homes, ensuring quality care while enjoying exceptional flexibility.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Ability to design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members
  • Thoroughly evaluate medical histories, current conditions, and medication profiles
  • Provide clear education to patients about their conditions and medication regimen
  • Identify gaps in care, supplying actionable recommendations for improvement
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Holds an active Nurse Practitioner license in North Carolina
  • Board certified via AANP or ANCC
  • Current CPR certification (BLS)
  • Graduate of an accredited MSN program
  • At least 6 months recent NP experience (highly desirable)
