Job Description
Take on a dynamic, patient-focused role conducting home health risk assessments for Medicare Advantage members across Wake County, including Raleigh, Cary, and Apex. As a Nurse Practitioner in this per diem position, youll use your expertise to deliver comprehensive evaluations in patients homes, ensuring quality care while enjoying exceptional flexibility.
Schedule:
- Flexible shifts offered Monday through Sunday
- Ability to design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Thoroughly evaluate medical histories, current conditions, and medication profiles
- Provide clear education to patients about their conditions and medication regimen
- Identify gaps in care, supplying actionable recommendations for improvement
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Holds an active Nurse Practitioner license in North Carolina
- Board certified via AANP or ANCC
- Current CPR certification (BLS)
- Graduate of an accredited MSN program
- At least 6 months recent NP experience (highly desirable)
