Job Description

Deliver focused and compassionate patient care by conducting home health risk assessments for Medicare Advantage members within Johnston County, NC. In this per diem nurse practitioner position, you will leverage your clinical skills to support patients in their own homes, performing comprehensive evaluations and providing essential education.

Schedule

  • Flexible shifts offered between Monday and Sunday
  • Create your own work schedule
  • Weekly or bi-weekly minimum hourly commitment

Core Responsibilities

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan participants
  • Review and evaluate medical history, chronic and acute conditions, and medication usage
  • Educate patients and caregivers regarding diagnoses and medication regimens
  • Identify gaps in care and communicate recommendations to enhance patient health
  • Visits typically last around 45 minutes
  • No responsibility for treatment plan creation or medication prescribing

Qualifications

  • Active Nurse Practitioner license in North Carolina
  • Board Certification via AANP or ANCC
  • Current CPR (BLS) certification
  • Master of Science in Nursing (MSN) from an accredited institution
  • Minimum of 6 months' recent experience as a Nurse Practitioner preferred
