Bring your expertise as a Nurse Practitioner to an impactful home health setting, providing risk assessments for Medicare Advantage plan members across Columbia County. This per diem opportunity empowers you to create your own schedule, supporting work-life balance while delivering essential healthcare services.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Options to select your hours and arrange shifts to fit personal commitments
- Choose minimum weekly or bi-weekly commitments that work best for you
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patient homes (approximately 45 minutes per evaluation)
- Thoroughly review medical history, current conditions, and medication use
- Offer education about health status, disease management, and medications to patients and families
- Identify care gaps and suggest actionable recommendations for follow-up
- Visits are focused on assessmentthere is no requirement for treatment plan development or prescribing
Qualifications:
- Active Nurse Practitioner license in Oregon
- Current BLS certification (CPR)
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent experience as a Nurse Practitioner preferred
