Job Description

Play a vital role in supporting individuals health across Suffolk County, Massachusetts, by conducting home health risk assessments as a Nurse Practitioner. This per diem opportunity is ideal for those who value flexibility, autonomy, and meaningful patient interaction. As part of this role, you will perform in-home assessments with Medicare Advantage members living in areas such as Boston, Revere, and Chelsea. Each appointment takes approximately 45 minutes and may include annual wellness evaluations or post-discharge follow-ups.

  • Minimum weekly or bi-weekly hourly commitments for continual engagement

Key Responsibilities:

  • Provide in-home annual health risk assessments for Medicare Advantage plan members
  • Thoroughly evaluate prior/current medical histories, chronic conditions, and medications
  • Offer education about diagnoses and prescribed treatments
  • Identify gaps in care and deliver actionable recommendations (no requirement to prescribe medications or set up treatment plans)
  • Ensure high quality visits focused on safety, comfort, and dignity

Qualifications:

  • Active Nurse Practitioner license in Massachusetts
  • Board certification (AANP or ANCC)
  • Current BLS/CPR certification
  • MSN from an accredited school of nursing
  • Minimum of six months recent NP experience preferred

This role provides a competitive rate of $80$90 per completed evaluation, as well as a mileage stipend and compensation for administrative duties.
