Job Description

Deliver high-quality care where it matters mostat the patients home. This per diem Nurse Practitioner role focuses on conducting home health risk assessments for Medicare Advantage members across Middlesex County, including cities such as Cambridge, Lowell, and Newton. Each assessment averages 45 minutes and may occur as an annual health check or post-discharge evaluation. Youll play an important role in supporting patient health and wellness, with the flexibility to craft your own schedule.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Design your own work schedule
  • Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members
  • Collect and evaluate current and prior medical, medication, and health history
  • Educate patients about their health conditions and medication management
  • Identify and report gaps in care, providing tailored recommendations for improvement
  • Enjoy a role focused strictly on assessment and education; no need to prescribe or create treatment plans

Qualifications:

  • Valid Nurse Practitioner license in Massachusetts
  • Current Basic Life Support (BLS) CPR certification
  • Board Certification through AANP or ANCC required
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least six months of recent NP experience preferred
