Job Description

This per diem role offers a high degree of autonomy and flexibility within Adams County, including Quincy, Camp Point, and Payson. You'll have the chance to positively impact patient outcomes by identifying health risks and educating members in the comfort of their homes.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Design your own schedule to fit your preferences
  • Minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct comprehensive home health risk assessments (approx. 45 minutes each)
  • Assess prior and current medical history, health conditions, and medication usage
  • Educate patients on their diagnoses and medications, reinforcing self-management strategies
  • Identify and address gaps in care, offering actionable recommendations
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner License in Illinois
  • Current BLS/CPR certification
  • Board certification from AANP or ANCC
  • MSN degree from an accredited institution
  • At least 6 months of current Nurse Practitioner experience preferred
