Schedule:
- Flexible shifts available Monday through Sunday
- Autonomy to design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct 45-minute home health risk assessments for Medicare Advantage plan members
- Evaluate patients prior and current medical history, chronic conditions, and medications
- Deliver education regarding medical conditions and prescribed medications
- Identify gaps in care and recommend solutions to improve health outcomes
- No requirement to create ongoing treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification by AANP or ANCC
- Valid CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
