Take on a critical role by delivering in-home health risk assessments for Medicare Advantage members across Saratoga County. This per diem opportunity allows Nurse Practitioners to work autonomously, utilizing their expertise in locations such as Saratoga Springs, Clifton Park, and Halfmoon, ensuring high-quality patient care right where its needed.

Schedule:

• Flexible shifts available seven days a week
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual health risk assessments in patients homes
• Review and evaluate medical history, ongoing conditions, and medication regimens
• Offer patient education regarding health conditions and prescribed medications
• Detect and address potential gaps in care, providing recommendations for further action
• Focus on assessmentsno need to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in New York
• Current BLS CPR certification
• Board Certification through AANP or ANCC required
• Minimum of 6 months NP experience preferred
• MSN degree from an accredited nursing institution

This position offers $80$90 per completed evaluation, plus a mileage stipend and additional compensation for administrative time, enabling you to maximize earnings while supporting your community.
