Step into an essential clinical role conducting home health risk assessments for Medicare Advantage plan members across Saratoga County, NY. As a certified Nurse Practitioner, you will have the flexibility to set your own per diem schedule, delivering quality care directly in patients homes within communities including Clifton Park, Saratoga Springs, and Halfmoon.

Schedule:

• Flexible shifts offered Monday through Sunday
• Create your own schedule to suit your lifestyle
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for members of a Medicare Advantage plan
• Review and document prior and current medical history, medical conditions, and medication regimens
• Educate patients on medical conditions and the appropriate use of medications
• Clearly identify care gaps and provide actionable recommendations for patients and families
• No prescribing of medications or development of treatment plansfocus is on assessment and education

Qualifications:

• Valid Nurse Practitioner license in the State of New York
• Current CPR (BLS) certification
• Board Certification via AANP or ANCC required
• Minimum of 6 months of recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution

This per diem opportunity rewards each completed evaluation with competitive compensation, including a stipend for mileage and administrative time, paid at between $80 and $90 per evaluation.
