Job Description
Take your clinical expertise into the homes of Medicare Advantage plan members as a Nurse Practitioner specializing in home health risk assessments. In this per diem role, you will visit individuals throughout Saratoga County, including areas such as Clifton Park, Saratoga Springs, and Halfmoon, to conduct comprehensive health evaluations and support patients well-being.
Schedule:
- Flexible scheduling available Monday through Sunday
- Set your own shifts for optimal work-life balance
- Satisfy a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete detailed annual or post-discharge home health risk assessments, with each visit averaging approximately 45 minutes
- Review, document, and update medical histories, current conditions, and medication lists
- Provide patient education regarding chronic and acute conditions and proper medication management
- Identify care gaps and deliver actionable recommendations to improve health outcomes
- Assess, rather than treatno medication prescribing or treatment planning required
Desired Qualifications:
- Nurse Practitioner license in the state of New York
- Successful completion of an MSN degree from an accredited nursing program
- Active board certification through AANP or ANCC
- Current CPR/BLS certification
- Six months or more recent nurse practitioner experience in a clinical setting is preferred
