Deliver high-impact care in the community by performing home health risk assessments for Medicare Advantage plan members in the Pullman area and throughout Whitman County. As a Nurse Practitioner in this per diem role, you will utilize your clinical expertise to support members well-being right in their homes. Youll have the flexibility to tailor your work schedule, choosing shifts between Monday and Sunday, and determine your own weekly or bi-weekly commitmentideal for work-life balance.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Set your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments on Medicare Advantage plan members
• Assess prior and current medical history, ongoing conditions, and medication use
• Provide tailored education regarding medical conditions and prescribed medications
• Identify gaps in care and recommend next steps for follow-up or management
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Current Nurse Practitioner license in Washington State
• Active BLS/CPR certification
• Board certified by AANP or ANCC
• At least six months of recent NP experience preferred
• MSN from an accredited nursing program
