Take on a pivotal role as a healthcare provider conducting home health risk assessments for Medicare Advantage plan members in Wythe County, VA. This per diem opportunity allows you to provide valuable clinical insight for patients residing in Wytheville, Rural Retreat, and surrounding areas through in-home evaluations.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to suit your lifestyle
- Minimum weekly or bi-weekly hourly commitment according to your availability
Key Responsibilities:
- Complete in-depth annual risk assessments for Medicare Advantage members in their homes
- Assess patients prior and current medical history, conditions, and medication usage
- Deliver clear education regarding diagnoses and prescribed medications
- Identify and communicate gaps in care, making recommendations for follow-up or resources as needed
- Maintain objectivityno need to create treatment plans or write prescriptions
Qualifications:
- Active Nurse Practitioner License in the state of VA
- Board Certification with AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum of six months recent experience as a Nurse Practitioner is preferred
