Step into a rewarding per diem opportunity as a Nurse Practitioner focusing on home health risk assessments in Oswego County. This role centers on conducting thorough in-person evaluations for Medicare Advantage plan members in their homes across areas such as Pulaski, Fulton, and Brewerton. Each assessment generally takes about 45 minutes, ranging from annual health check-ups to post-discharge visits. Flexibility is a key feature of this position, allowing practitioners to determine their own schedules and shift preferences.
Schedule:
- Flexible shifts available between Monday and Sunday
- Choose your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual risk assessments for Medicare Advantage members
- Review patients medical and medication histories to evaluate current health conditions
- Identify gaps in care and make informed recommendations for improvement
- Educate members about their health conditions and prescribed medications
- Travel to member homes within Oswego County for assessments
- No requirement for treatment planning or prescribing medications
Qualifications:
- Active Nurse Practitioner License in New York
- Board Certification through AANP or ANCC
- Valid CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
This per diem position offers competitive earnings between $80 and $90 per completed evaluation, in addition to a mileage stipend for travel and compensation for administrative time.
