Seeking an experienced Nurse Practitioner to conduct home health risk assessments for Medicare Advantage plan members within Oswego County, including the areas of Fulton, Brewerton, and Pulaski. This per diem opportunity is ideal for professionals who value flexibility and autonomy while making a positive impact on patients lives.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Options for minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage members
- Review patient medical histories, current conditions, and medication coverage during each visit (average interaction: 45 minutes)
- Provide education regarding medical conditions, medications, and preventive healthcare
- Identify and document gaps in care, offering appropriate recommendations for improvement
- No treatment plan development or medication prescribing is required
Qualifications:
- Active Nurse Practitioner License in New York
- Current CPR certification (BLS)
- Board Certified by AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of recent NP experience preferred
