Deliver vital care directly where it matters mostat home. We are seeking skilled Nurse Practitioners to perform comprehensive home health risk assessments for Medicare Advantage members across Princeton, Bluefield, Athens, and Kanawha County. This per diem opportunity provides unmatched flexibility, autonomy, and meaningful patient impact.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Self-select your schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual, post-discharge, or episodic home health risk assessments in patient homes
• Assess and document prior/current medical history, ongoing conditions, and medication use
• Educate patients about diagnoses, wellness, and medication adherence
• Identify gaps in preventive care and recommend next steps
• No requirement to prescribe medications or create ongoing treatment plans

Qualifications:

• Active Nurse Practitioner license in West Virginia
• Board certification by either AANP or ANCC
• Current BLS (CPR) certification
• Masters degree in Nursing (MSN) from an accredited program
• At least six months experience as a Nurse Practitioner (preferred)

This role offers compensation of $80$90 per evaluation, plus a mileage stipend and additional consideration for administrative time.
