Shifts are offered with maximum flexibilitychoose your own days and hours to fit within your lifestyle.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedulecoordinate directly for preferred days/hours
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual in-home health risk assessments, averaging 45 minutes per visit
• Assess patients current and prior medical histories, conditions, and medication usage
• Educate individuals on health conditions, preventive practices, and medication regimens
• Detect care gaps and provide actionable recommendations (no prescribing or treatment plan development required)
• Document findings with accuracy and professionalism per assessment protocols

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Current CPR (BLS) certification
• Board certification via AANP or ANCC
• Minimum of 6 months Nurse Practitioner experience preferred
• MSN from an accredited nursing program

Compensation for this role includes $80-$90 per completed evaluation, along with a mileage stipend and payment for administrative time.
