Job Description

Take on a dynamic and meaningful per diem opportunity as a Nurse Practitioner specializing in home health risk assessments across Bonner County. Serve diverse communities, including Sandpoint, Ponderay, and Priest River, and play a key role in supporting the health and well-being of Medicare Advantage plan members in the comfort of their own homes.

Schedule:

  • Flexible shifts available Monday through Sunday

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Evaluate patients medical histories, current conditions, and medication regimens
  • Deliver clear education and counseling on diagnoses and prescriptions to patients
  • Identify any care gaps and suggest recommendations for follow-up action
  • No ongoing treatment planning or prescriptions requiredfocus on thorough, high-quality assessments
  • Each home visit typically lasts around 45 minutes and may include post-discharge evaluations

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current CPR certification (BLS required)
  • Board certification through AANP or ANCC
  • At least 6 months of recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program

Compensation for this position is competitive, and you will receive $80$90 per evaluation, along with a mileage stipend and administrative time allowance.
