Step into a dynamic per diem role, delivering expert healthcare directly to patients homes in Hempstead County. As a Nurse Practitioner specializing in home health, youll play an integral part in improving community health by conducting comprehensive risk assessments for Medicare Advantage members. Your routine will be varied, including annual health reviews and post-discharge evaluations, with each visit typically lasting about 45 minutes. Locations may include Hope, Blevins, and Perrytown, with flexibility in your daily itinerary.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling allows you to create your own workweek
  • Commit to a minimum weekly or bi-weekly hourly requirement

Key Responsibilities:

  • Complete thorough home health risk assessments for Medicare Advantage members
  • Assess patients medical and medication histories, as well as ongoing medical conditions
  • Educate patients on health conditions and medication management
  • Identify and close gaps in care by providing actionable recommendations
  • No required prescribing or treatment plan management

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR (BLS) certification
  • Board Certification via AANP or ANCC
  • Minimum 6 months recent experience as a nurse practitioner preferred
  • Masters degree in Nursing (MSN) from an accredited institution

This role offers compensation of $80$90 per completed evaluation, with an added mileage stipend and administrative time reimbursement.
