Job Description
Deliver advanced patient care in a community-focused role conducting home health risk assessments for Medicare Advantage members. This per diem opportunity offers autonomy and flexibility, allowing you to perform meaningful clinical evaluations with a personalized approach. Assessments typically last around 45 minutes and may include annual health reviews or post-discharge visits within Washington County, Arkansas, serving residents in Fayetteville, Springdale, Farmington, and nearby areas. Assignments will be based throughout these locations on a shift-by-shift basis.
Schedule:
- Choose flexible shifts Monday through Sunday
- Build your own schedule to fit your personal commitments
- Minimum weekly or bi-weekly hours required
Key Responsibilities:
- Perform comprehensive home-based risk assessments for Medicare Advantage participants
- Review and evaluate current and previous medical histories, chronic conditions, and medication profiles
- Offer clear, relevant education to patients about their medical issues and prescribed therapies
- Identify gaps in care and deliver recommendations to help resolve them
- Focus strictly on assessment and educationno need for developing treatment plans or prescribing medications
Qualifications:
- Nurse Practitioner license in Arkansas
- Active BLS (CPR) certification
- Board certification from AANP or ANCC
- At least 6 months of current Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
