Job Description

Take on a dynamic Nurse Practitioner role focused on delivering home health risk assessments to Medicare Advantage members in Lincoln County, OK. This per diem opportunity offers unmatched flexibilitycreate a schedule that fits your life, with shifts available any day of the week. Assignments may be in various Lincoln County communities, such as Chandler, Stroud, or Prague. Each in-home assessment averages 45 minutes and may involve annual wellness and post-discharge visitsno treatment planning or prescribing required.

Schedule:

  • Flexible shifts from Monday to Sunday
  • Self-schedulingdesign your own hours
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members
  • Review and document patients medical history, current conditions, and medications
  • Offer patient education about health conditions and medication management
  • Pinpoint care gaps and provide recommendations
  • Ensure accurate, timely completion of evaluation documentation
  • No responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Board Certification by AANP or ANCC
  • Valid CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • Minimum six months' experience as a Nurse Practitioner preferred
