Job Description

Deliver essential care in the comfort of patients' homes by performing impactful home health risk assessments throughout Seminole County, OK. As a Nurse Practitioner focused on home health, you will play a key role in evaluating and educating Medicare Advantage plan members in communities such as Wewoka, Bowlegs, and Konawa. This per diem assignment offers significant professional flexibility, allowing you to tailor your work to your lifestyle and availability.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Set your own schedule
  • Options for minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review and evaluate patients medical history and existing conditions
  • Assess medication history and efficacy
  • Educate members on their medical conditions and proper medication usage
  • Identify gaps in care and recommend supportive resources or services
  • No treatment planning or prescription writing is required

Qualifications:

  • Current Nurse Practitioner license in Oklahoma
  • Active CPR certification (BLS)
  • Board certified through AANP or ANCC
  • MSN from an accredited nursing program
  • At least six months of recent experience as a Nurse Practitioner preferred
