Exciting per diem opportunity available for a Nurse Practitioner with experience in home health risk assessments. This role focuses on providing high-quality, patient-centered care for Medicare Advantage members through comprehensive evaluations conducted in the comfort of patients homes across Knox County. Youll play a vital part in improving patient outcomes while enjoying the flexibility to design your own work schedule.

- Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members in Farragut, Powell, and surrounding areas
- Thoroughly evaluate prior/current medical history, chronic conditions, and medication regimens
- Offer patient education on diagnoses and medications to support health literacy
- Identify and address gaps in care, providing actionable recommendations
- Role does not involve prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS CPR certification
- Board certified by AANP or ANCC
- At least six months of recent Nurse Practitioner experience (preferred)
- Masters degree in Nursing (MSN) from an accredited institution
This per evaluation role offers compensation of $80$90 per completed assessment, with additional mileage reimbursement and compensation for administrative time.
