Deliver high-quality patient-centered care as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, you will travel to homes throughout Douglas County, GA, including areas such as Douglasville, Villa Rica, and Austell. Your focus will be on evaluating Medicare Advantage plan members, adapting to a variety of assessment typesincluding annual health reviews and post-discharge visits. Each home visit typically lasts about 45 minutes and does not require prescribing medications or developing treatment plans.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-schedulingdesign your preferred weekly or bi-weekly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review and evaluate patients previous and current medical history, health conditions, and medication use
• Deliver patient education regarding medical conditions and medication management
• Identify care gaps and provide actionable recommendations for improvement
• Complete documentation and administrative tasks associated with each visit

Qualifications:

• Active Nurse Practitioner license in Georgia
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• At least 6 months of recent experience as a Nurse Practitioner preferred
• Master's degree in nursing (MSN) from an accredited institution
