Job Description

Advance your clinical skills in a flexible per diem role, providing essential care to patients in their homes. This position offers the chance to conduct home health risk assessments for Medicare Advantage members across Douglas County, GA, including Douglasville, Villa Rica, and Austell. Each assessment typically takes about 45 minutes and may cover annual health reviews or post-discharge follow-ups. As a provider, youll have autonomy over your schedule while playing a critical part in patient wellness.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment required

Core Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review prior and current medical history, conditions, and medications
  • Educate patients on chronic condition management and medication usage
  • Identify care gaps and recommend actions to address them
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Georgia
  • Current CPR/BLS certification
  • Board certification through AANP or ANCC
  • Minimum of 6 months of recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution

This per diem opportunity offers compensation of $80$90 per evaluation, along with a mileage stipend and payment for administrative time.
