Advance your career as a Nurse Practitioner specializing in home health risk assessments within Dutchess County, NY. In this per diem position, youll engage directly with Medicare Advantage plan members in their homes, providing comprehensive annual or post-discharge assessments that typically last about 45 minutes each. Your assessments may take place in Poughkeepsie, as well as surrounding areas such as Beacon, Fishkill, and Rhinebeck.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform in-home risk assessments for Medicare Advantage plan members
• Review and evaluate patients prior and current medical history, ongoing conditions, and medication regimens
• Educate individuals on their health conditions and prescribed medications
• Identify care gaps and offer actionable recommendations for improvement
• Note: This role does not require prescribing medications or developing treatment plans

Qualifications:

• Active Nurse Practitioner license in New York
• Current CPR (BLS) certification
• Board certified through AANP or ANCC
• 6+ months of experience as an NP (preferred)
• Master of Science in Nursing from an accredited program
