Job Description

Make a difference in patient care by conducting home health risk assessments for Medicare Advantage plan members throughout Dutchess County, NY. This per diem opportunity is designed for Nurse Practitioners who thrive in an autonomous environment and value flexibility. You'll provide essential assessments, including annual health reviews and post-discharge visits, in patients homes across Poughkeepsie, Beacon, Fishkill, Rhinebeck, and surrounding areas.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Option to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, evaluate medical conditions, and document medication history
  • Deliver patient education regarding health conditions and medication regimens
  • Identify and communicate gaps in care, offering actionable recommendations for improvement
  • Assessment visits onlyno requirement for developing treatment plans or prescribing medication

Qualifications:

  • Current Nurse Practitioner license in New York State (NY)
  • Active CPR certification (BLS)
  • Board certified through AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited program

This position offers competitive per evaluation compensation ranging from $80 to $90, with a mileage stipend and additional administrative time provided.
