Exceptional opportunity for a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members within St. Lawrence County, New York. This per diem role offers a high degree of flexibility, making it ideal for providers seeking to manage their schedules while making a meaningful impact in the community. Assessments typically take 45 minutes and may involve annual health evaluations or post-discharge visits. Locations include Potsdam, Massena, Ogdensburg, and Canton, with candidates potentially scheduled in any of these areas during their shifts.

Schedule:

• Flexible shifts, available Monday through Sunday
• Set your own hours and create a personalized schedule
• Minimum weekly or bi-weekly hourly commitment as needed

Primary Responsibilities:

• Complete comprehensive in-home health risk assessments for Medicare Advantage members
• Review and document prior and current medical history, conditions, and medication use
• Educate members on their health conditions and prescribed medications
• Identify gaps in care and offer practical recommendations
• No prescribing or treatment plan development required

Qualifications:

• Active Nurse Practitioner license in New York
• MSN degree from an accredited nursing program
• Current CPR (BLS) certification
• Board Certification (AANP or ANCC)
• At least six months of NP experience preferred

Compensation:

• Earn $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time
