Job Description
Make a significant impact in patient care by performing home health risk assessments as a Nurse Practitioner specializing in home health. In this per diem position, you will travel to patients' homes throughout Potsdam and the greater St. Lawrence County areaincluding Massena, Ogdensburg, and Cantonto evaluate the health status of Medicare Advantage plan members. With flexible, self-directed scheduling, this role is ideal for professionals who value autonomy and the opportunity to create their own work-life balance.
Schedule:
- Flexible shifts available between Monday and Sunday

- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Perform comprehensive annual or post-discharge health risk assessments in patients' homes
- Review and evaluate prior and current medical histories, conditions, and medications
- Educate patients about their health conditions and medication regimens
- Identify gaps in care and recommend strategies for improved health outcomes
- No responsibility for developing treatment plans or prescribing medicationsfocus is purely on assessment and education
Qualifications:
- Active Nurse Practitioner license in New York
- Board Certification from AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR (BLS) certification
- At least six months of nurse practitioner experience (preferred)
In this role, you will be compensated $80$90 per completed evaluation, along with a mileage stipend and reimbursement for administrative time.
