Key Responsibilities:
- Complete annual home health risk assessments, typically lasting 45 minutes per visit
- Evaluate patient medical histories, current conditions, and medication usage
- Educate patients about their health conditions and the importance of adhering to medication regimens
- Recognize care gaps and offer recommendations to close them
- Focus solely on assessmentno need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New York State
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
