Key Responsibilities:

  • Complete annual home health risk assessments, typically lasting 45 minutes per visit
  • Evaluate patient medical histories, current conditions, and medication usage
  • Educate patients about their health conditions and the importance of adhering to medication regimens
  • Recognize care gaps and offer recommendations to close them
  • Focus solely on assessmentno need to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN from an accredited nursing program
  • At least six months of recent Nurse Practitioner experience preferred
