Seeking a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members within Latah County, ID. This per diem role offers the flexibility to create your own schedule, with shifts available throughout the week in locations including Moscow, Genesee, and Troy. As a Nurse Practitioner in this position, you will conduct visits that typically last around 45 minutes, focusing on annual health evaluations or post-discharge assessments, depending on patient needs.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Review and evaluate patients prior/current medical history, conditions, and medication use
• Educate patients on their health conditions and proper medication management
• Detect gaps in care and offer recommendations to resolve them
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner License in Idaho
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• At least 6 months of nurse practitioner experience preferred
• MSN degree from an accredited nursing program

This opportunity provides competitive compensation at $80-$90 per evaluation, along with a mileage stipend and administrative time.
