Advance your nursing expertise in a dynamic per diem opportunity focused on home health risk assessments across Latah County. This role supports Medicare Advantage plan members through individualized, in-home visitseach lasting about 45 minutesproviding either annual health assessments or post-discharge follow-up. The position offers extensive flexibility, with visits scheduled in communities throughout the area.
Schedule:

Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Review prior and current medical history, chronic and acute medical conditions, and medication usage
- Offer education regarding diagnoses and medications
- Identify and document any gaps in care while recommending solutions to close those gaps
- Please note: this position does not involve creating treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in Idaho
- Current CPR certification (BLS required)
- Board certification via AANP or ANCC
- Minimum of 6 months of recent experience as a Nurse Practitioner preferred
- MSN from an accredited nursing program
