Advance your nursing expertise in a dynamic per diem opportunity focused on home health risk assessments across Latah County. This role supports Medicare Advantage plan members through individualized, in-home visitseach lasting about 45 minutesproviding either annual health assessments or post-discharge follow-up. The position offers extensive flexibility, with visits scheduled in communities throughout the area.

Schedule:

• Flexible shifts available seven days a week

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review prior and current medical history, chronic and acute medical conditions, and medication usage
• Offer education regarding diagnoses and medications
• Identify and document any gaps in care while recommending solutions to close those gaps
• Please note: this position does not involve creating treatment plans or prescribing medications

Qualifications:

• Valid Nurse Practitioner license in Idaho
• Current CPR certification (BLS required)
• Board certification via AANP or ANCC
• Minimum of 6 months of recent experience as a Nurse Practitioner preferred
• MSN from an accredited nursing program
