Job Description

Deliver crucial healthcare services directly to patients homes as a Nurse Practitioner specializing in home health risk assessments in Kootenai County, ID. This per diem opportunity allows professionals to make a meaningful impact by supporting and educating Medicare Advantage plan members following hospital discharges or during annual health reviews.

Schedule:

  • Flexible shifts available seven days a week
  • Self-scheduling for maximum convenience
  • Minimum hourly commitment per week or bi-weekly, based on availability

Responsibilities:

  • Conduct comprehensive home health risk assessments in Post Falls, Coeur dAlene, Hayden, and surrounding areas within Kootenai County
  • Complete evaluations by reviewing medical histories, current conditions, and medication usage
  • Educate patients about diagnosed medical conditions and prescribed medications to encourage understanding and self-care
  • Identify and document gaps in care, recommending ways to optimize health outcomes
  • These visits, lasting approximately 45 minutes, do not require developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner License in Idaho
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least six months of recent Nurse Practitioner experience is preferred

Compensation: Earn $80$90 per evaluation, with additional stipends for mileage and administrative time.
