Step into a dynamic, patient-centered role providing home health assessments for Medicare Advantage members across Scioto County, Ohioincluding Portsmouth, Wheelersburg, New Boston, and Minford. As a key healthcare provider, you will leverage your expertise as a Nurse Practitioner to perform comprehensive risk evaluations and support members overall well-being from the comfort of their own homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to accommodate personal and professional goals
- Minimum weekly or bi-weekly hourly commitments to suit your preferences
Primary Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage health plan participants
- Gather and evaluate prior and current medical history, ongoing medical conditions, and medication histories during each visit (approximately 45 minutes per assessment)
- Deliver patient education on health conditions, medication use, and preventive strategies tailored to individual needs
- Identify care gaps, providing actionable recommendations to encourage optimal health outcomes
- Focus solely on risk assessmentno treatment planning or prescription responsibilities involved
Desired Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR (BLS) certification
- At least six months of recent Nurse Practitioner experience preferred
