Deliver vital care while enjoying the freedom of a per diem schedule as a Nurse Practitioner specializing in home health risk assessments. Serve Medicare Advantage plan members throughout Scioto County, including Portsmouth, Wheelersburg, New Boston, and Minford, by providing compassionate, high-quality assessments in the comfort of their own homes. Each evaluation typically takes about 45 minutes, focusing on preventive care and comprehensive patient understanding, without the responsibility of creating treatment plans or prescribing medications.

Schedule:

• Flexible shifts available between Monday and Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct thorough annual health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, chronic and acute conditions, and medication use
• Offer clear education, guidance, and resources on conditions and medications
• Identify care gaps, delivering actionable recommendations to enhance patient outcomes
• Visits may involve annual wellness checks or post-discharge follow-ups

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR certification (BLS)
• Board certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least six months of recent experience as an NP is preferred

This position offers $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
