Deliver vital care directly to patients homes as an experienced Nurse Practitioner specializing in home health risk assessments on a per diem basis. This role offers a high degree of scheduling flexibility, allowing you to design your own shifts between Monday and Sunday throughout Strafford County, NHincluding areas like Dover, Rochester, and Somersworth. Each home visit will average 45 minutes and involve comprehensive health risk assessments for Medicare Advantage plan members, with the type of visit varying between annual assessments and post-discharge check-ins.

Schedule:

• Choose your own shifts between Monday and Sunday
• Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage plan participants in Strafford County
• Review and evaluate medical history, current conditions, and medication lists
• Provide patient education about diagnoses and therapies
• Identify gaps in patient care, offering recommendations for follow-up or intervention
• No requirement to prescribe medications or implement treatment plans

Qualifications:

• Active Nurse Practitioner license in NH
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• Six months of recent NP experience preferred
