Providing in-home and senior living care with a focus on comprehensive, patient-centered treatment, this role supports insurance members by delivering primary, preventative, urgent, and acute care services designed to reduce unnecessary hospitalizations. The position emphasizes conducting thorough health assessments at least monthly, or more frequently as needed, to ensure effective management and timely intervention in the home setting.
Schedule:
- Monday to Friday, 8 AM 5 PM
- No weekends or holidays
Key Responsibilities:
- Develop detailed problem lists and individualized medical treatment plans for each patient
- Deliver primary, preventive, urgent, or acute care directly on-site within patients homes or senior living communities
- Identify and stratify high-risk patients for increased clinical monitoring and frequent visits
- Establish contingency plans tailored to manage high-risk conditions effectively
- Collaborate thoroughly with primary care physicians and facility staff, including Directors of Nursing, to coordinate patient care
- Provide bedside care, devise care plans, and create medical orders in collaboration with multidisciplinary teams
- Conduct single visits for patients recently discharged from skilled nursing facilities as part of their transition plan
Qualifications and Desired Experience:
- Master of Science in Nursing (MSN) degree
- Current Oregon Nurse Practitioner license and DEA certification
- Basic Life Support (BLS) certification
- Experience in home health, hospice, or related fields preferred
- Proven ability to manage care in ambulatory and home settings
