Step into a crucial patient-facing position as a Nurse Practitioner specializing in home health assessments. This per diem role is designed for a provider who values both flexibility and meaningful patient interaction. You'll serve Medicare Advantage plan members across Portland and neighboring communities such as South Portland and Westbrook, conducting comprehensive home health risk assessments. Each visit, averaging 45 minutes, allows you to evaluate patient histories, identify care gaps, and offer dedicated health educationwithout the need to create treatment plans or prescribe medications.

Key Responsibilities:

• Perform in-home annual health risk assessments for Medicare Advantage members
• Review medical histories, current conditions, and medications
• Deliver tailored health education during each visit
• Identify care gaps, providing recommendations for follow-up
• Complete post-discharge and annual health evaluations as assigned

Qualifications:

• Active Nurse Practitioner license in the state of Maine
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification required
• MSN from an accredited nursing program
• At least six months NP experience is strongly preferred
