Job Description

Elevate your career as a Nurse Practitioner supporting home health services with flexible, per diem shifts serving Medicare Advantage members across Kalamazoo County. In this vital position, you'll deliver comprehensive home health risk assessments, directly impacting the well-being of patients in their homes, including areas such as Kalamazoo, Portage, and Oshtemo.

Schedule:

  • Flexible shifts available Monday through Sunday
  • The ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitments required

Primary Responsibilities:

  • Conduct detailed annual home health risk assessments with Medicare Advantage plan members
  • Review and evaluate patient medical history, existing conditions, and medication use
  • Educate members about their health conditions and prescribed medications
  • Identify and address care gaps, recommending actionable solutions to improve patient outcomes
  • Visits designed for assessments onlyno prescribing or development of treatment plans required
  • Each assessment typically takes about 45 minutes
  • Potential visits include annual health assessments and post-discharge evaluations

Qualifications:

  • Active Nurse Practitioner license in Michigan
  • Current Basic Life Support (BLS) certification
  • Board Certification through AANP or ANCC
  • At least 6 months of recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program

Compensation for this opportunity ranges from $80-$90 per evaluation, plus a mileage stipend and coverage for administrative time, allowing you to maximize your earnings on a flexible schedule.
